Ken Kaneki es un estudiante universitario del ratón de biblioteca que resuelve a una muchacha nombra a Rize en un café que él frecuenta. Tienen la misma edad y tienen los mismos intereses, por lo que rápidamente se acercan. Poco sabe Kaneki que Rize es un ghoul - una especie de monstruo que vive cazando y devorando carne humana. Cuando parte de su órgano especial - "el niño rojo" - es trasplantado a Kaneki, se convierte en un ghoul mismo, atrapado en un mundo deformado donde los seres humanos no son la parte superior de la cadena alimentaria.
